Protected Health Information and HIPAA

Certain health-related information may qualify as Protected Health Information, commonly known as PHI, when it is created, received, maintained, or transmitted in a capacity regulated by the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations, commonly known as HIPAA.

This Privacy Statement does not represent that Homefront Honor Foundation is currently a HIPAA-covered entity or business associate, or that every item of health-related information received by HHF constitutes PHI under HIPAA.

If HHF becomes subject to HIPAA through its programs, contracts, partnerships, activities, or business-associate relationships, HHF will implement the privacy, security, breach-notification, documentation, authorization, training, contractual, and information-handling requirements applicable to that relationship.

Where required, HHF may enter into a Business Associate Agreement or other appropriate privacy and data-protection agreement before creating, receiving, maintaining, or transmitting PHI on behalf of a HIPAA-regulated organization.

Regardless of whether particular information is legally classified as PHI, HHF intends to treat individually identifiable health information as sensitive and restricted information.

Access will be limited to authorized individuals with a legitimate need to know, and reasonable administrative, technical, and physical safeguards will be used to protect the information.

When you request assistance connecting with another organization, HHF may ask for permission to share limited information necessary to make or coordinate the referral.

Whenever appropriate, HHF will seek authorization before disclosing nonpublic information to an unrelated referral organization.

Third-party organizations maintain their own privacy practices, eligibility rules, security procedures, and records.

HHF is not responsible for the independent privacy practices of another organization after information is lawfully provided to that organization.

11. Cookies and Website Technologies

Our website may use cookies or similar technologies for security, functionality, preferences, analytics, performance, fraud prevention, or website administration.

The technologies used may change as website features are added, removed, enabled, or disabled. Certain cookies may be necessary for the website to function properly.
