You may submit a request so our team can better understand the situation, identify possible resources, and determine whether any current or future support option may be relevant. Submission does not guarantee funding, eligibility, placement, services, or acceptance into a program.

Tell us what is needed—not everything that has ever happened.
A general assistance request should not include Social Security numbers, passwords, full banking information, medical records, military records, benefit-account credentials, full birth dates, or copies of identity documents.
If a future program requires documentation, the foundation should first explain why it is needed, how it will be transmitted, who may access it, and how long it will be retained.
